Directions:

Completed forms must be presented prior to taking a physical.  Your child can bring the forms with them to the physical or they may be brought to the athletic office (Room 184).  If the forms are not completed, the doctor will not perform the physical.  You must complete a Yellow Player-Parent Agreement, Blue Health History Questionnaire (Part A) and White Consent to Random Testing in order to take a physical by the school doctor.

If you choose to have a physical done by your own doctor, physical form (Part B) must be completed by the doctor and returned to the nurse's office in the high school along with the 3 other forms.

Please note that the Player-Parent Agreement and Health History forms are only for one sport; if your child plans to participate in another season in addition, another set of forms will have to be completed at the beginning of the other season(s).  However, the physical is good for one full year.
